Liposuction / Liposculpture is the removal of unwanted buldges caused by excess localized fatty deposits. Liposuction alone does not significantly improve cellulite. A small diameter stainless steel suction tube (cannula) is inserted through one or more tiny, inconspicously placed incisions. This cannula is attached via tubing to a fat collection container, which is powered by a high pressure suction device (vacuum). Moving the cannula back and forth breaks up the fat; and with accompanying suction removes the unwanted fat, permitting liposculpting.

Early, vintage forms of liposuction were not always safe. However, modern liposuction is not only safe, but one of the commonly performed and widely practiced cosmetic procedures by cosmetic dermatologic surgeons; thanks to the pivotal development of Tumescent Liposuction by cosmetic dermatologic surgeon Arnold Klein in 1985. Published in November, 2002 in the prestigious Journal of Dermatologic Surgery, a landmark survey of 267 dermatologic cosmetic surgeons demonstrated no deaths in 66,570 liposuction procedures from 1994-2000.

Tumescent liposuction allows large volume / multiple area liposuction to be performed safely and painlessly under modified local anesthesia with decreased post-operative discomfort and recovery time, in an office-based surgery facility; and thus, eliminating the risk and expense of general anesthesia and the expense of hospital or outpatient surgical facilities. Patients receive minor sedation and/or pain medication to help them relax, yet are conscious, but pleasantly sedated and comfortable.

The differentiating feature of tumescent liposuction that has allowed modern liposuction to become so safe is the tumescent anesthetic technique. Tumescent anesthesia uses a large volume dilute anesthetic saline mixture, allowing large areas to be safely anesthetized for painless liposuction. Adrenaline added to the anesthetic solution reduce bruising and bleeding, a significant risk of traditional liposuction surgery of the past. Reduced bleeding and bruising speeds healing / recovery. Dilute sodium bicarbonate in the solution reduces the unpleasant stinging otherwise associated with the local anesthetic lidocaine within the solution, and thus, allows the injection of this mixture to proceed relatively painlessly.

The tumescent (or Klein) solution is injected in large volumes to cause the tissues to become firm and inflated (or tumescent). This allows our Dr. Robert Strimling, MD to suction extract the fat in a more accurate and uniform fashion, thereby producing smoother results. Instilling the large volume local anesthetic solution prior liposuction via very small, blunt-tipped cannulas also break-ups / “loosens” the fatty tissue; serving to facilitate its safe and uniform removal.

Laser Liposuction (e.g. SmartLipo, SlimLipo, ProLipo, Duolipo, Lipotherme, etc.) utilizes a laser fiber with tumescent anesthesia instead of a cannula to melt unwanted fat that still requires traditional suction removal. Laser liposuction is usually offered as an adjunct to tumescent liposuction; Laser liposuction may be used as a stand-alone procedure for small areas, such as the face and/or neck; where melted fat is absorbed without suction. Laser liposuction advocates claim laser derived heat may induce additional skin tightening, however, laser liposuction may also cause burns or punctures more easily. Laser liposuction alone or together with tumescent liposuction does not offer any significant advantages over tumescent liposuction alone.

Other variations of modern liposuction include:

  • (Internal) Ultrasound-assisted Liposuction (I-UAL) (VASER Lipo)

Ultrasound derived heat melts fat as in Laser liposuction. I-UAL is currenlty out of favor due to larger incision sites needed for cannula access and increased risk of burns and punctures due to heat generation.

  • Power-assisted Liposuction (PAL)
  • Body Jet (Water-assisted Liposuction)

Aesthetic results are comparable to traditional tumescent liposuction.

Many non-invasive, alternative fat busting technologies now exist. Historically, not as effective nor as practical as modern Liposuction, recent non-invasive technological improvements have now blurred this line
in the appropriate candidate. Non-surgical device-based fat destroying technologies include:
cryolipolysis, ultrasound, radiofrequency-based, and external laser and most recently, injection lipolysis with Kybella.

Perhaps, most well known is cryolipolysis or Coolsculpting. Zeltiq (now, Allergan) Coolsculpting (Cryolipolysis) requires multiple treatment sessions that require up to 1-2 hours each to treat each focal treatment zone. Currently, only a single focal area can be treated per session per device. Expect subtle (20-25% fat reduction) to slowly occur over several to multiple months and treatments. UltraShapes is a fat busting device that use cavitational ultrasound energy. VASER Shape is a less energetic / less effective version of UltraShapes. Liposonix uses ultrasound derived heat to destroy fat cells, but is far too painful to be practical and is generally not recommended. VelaShape combines IF (infrared), bi-polar RF (radiofrequency) and vacuum to melt fat, tighten skin and improve cellulite. Zerona combines external cold laser treatments with diet and exercise. iLipo XCell is another similar noninvasive external laser approach. Vanquish is a related external non-contact radiofrequency-based approach.

Of all the above methods, our preferred non-invasive liposuction alternative is Exilis monopolar radiofrequency, which is an FDA-approved, easy, safe, effective, painless fat disrupting / skin tightening treatment. Exilis also helps cellulite and can be used on almost any body part / area.

Tumescent liposuction still remains the gold standard in fat removal. However, liposuction is not a weight loss treatment nor does it significantly improve cellulite.
